Community Health Access & Referrals
Navigation support connecting community members to local screening, care, and social services partners across the Tampa Bay region.
Why this work
Navigation is the bridge between community and care.
Many community members face systemic barriers when trying to find affordable screening, behavioral health support, or social services. Our role is to help bridge that gap — explaining what's available, what to expect, and how to connect with trusted partner organizations.
We do not provide direct clinical services. Instead, we serve as a community-rooted navigator and educator, helping individuals understand and access the public health resources around them.
What we provide
Navigation and community resources
Screening Access Navigation
Information about where and how community members can access HIV, hepatitis, and STI screening through trusted public health and partner organizations in the Tampa Bay region.
Social Services Referrals
Warm referrals to partner organizations providing wrap-around support — food assistance, housing navigation, benefits enrollment, and other community resources.
Outreach Education
Community outreach events and presence in neighborhoods where information and trust matter most.
Partner Coordination
Collaboration with regional community-based organizations, public health departments, and healthcare partners to strengthen the local network of accessible support.
Service area
Serving the Tampa Bay region
Our community navigation and outreach reaches across the greater Tampa–St. Petersburg metropolitan area, with additional outreach connections in the Jacksonville-area corridor. Specific partner locations and current resource networks are provided on a case-by-case basis through community navigation.
